Many people are oh too familiar with the debilitating pain of gout. Gout occurs when body fluids get supersaturated with uric acid to the point that urate crystals form. These crystals often collect in the joints leading to a type of inflammatory arthritis that is commonly called ‘gout’.
Historically, gout was believed to be a disease of the upper class who could afford indulgent food and drink and a more leisurely / sedentary lifestyle than the working or lower classes. It used to be referred to as, morbus dominorum et dominus morborum – ‘the lord of disease and the disease of lords’. Interestingly, perhaps in an attempt to comfort its sufferers, the ailment also had a reputation as being an aphrodisiac!
‘Podagra’ is the term for the intense symptoms of gout in the first joint of the big toe – the usual place where it first appears. (One can remember this location by thinking of it as ‘big toe woe’.) A gout attack or flare most often begins overnight, so the sufferer wakes and wonders how they could have possibly injured their toe so badly in their sleep! Severe pain, swelling, redness, and extreme sensitivity are hallmarks of the condition. People often comment that the affected spot is so tender that even the touch of a bed sheet is painful. Movement of the joint is impeded by the pain, so mobility overall is usually affected, and sufferers often have trouble walking during an attack.
The first attack of gout is most often in the big toe (though it can be elsewhere), but subsequent attacks can involve multiple joints or different joints, especially the ankles, knees, elbows, wrists, and fingers.
One of the key features of gout is that the symptoms usually appear suddenly. The pain intensifies very quickly, and usually peaks within 4 to 24 hours of onset. The pain may remain severe for 1 to 2 days, and then begins to gradually dissipate. A typical ‘attack’ of gout lasts anywhere from 3 days to 2 weeks. The skin in the area of the gout flare may itch or peel as it starts to heal.
The pain is incredibly severe and the sufferer will not be able to ignore the symptoms. Chills and a low fever may accompany a gout attack as the body goes into an inflammatory response and the immune system attempts to remediate the uric acid crystal buildup. Tiredness and a general mild feeling of being unwell may also accompany the early stages.
After a gout attack resolves, the person is at increased risk of future attacks if lifestyle changes or treatments aren’t followed to decrease the amount of uric acid in their body. Subsequent gout attacks may last longer or be more severe than the initial one, with intervals between attacks varying. For some people, gout becomes a chronic issue, with attacks coming frequently and without complete resolution between them. If one attempts to address the root cause of gout before this stage, and reduce or avoid triggers, chronic gout can usually be averted.
Homeopathy can help to provide relief for the acute symptoms of gout as well as address the underlying causes and tendency to have gout attacks. During an acute gout attack, some of the following remedies may be useful, with the best results coming if the symptoms match the sufferer’s symptoms closely.
Ledum
This key remedy for gout is more likely to be needed in gout of the toes, feet, ankles, and knees than in upper extremities. There is often great swelling of the ball of the big toe. The gouty area will feel cold to the touch, but feels better for cold applications, not warm ones. So strong is this symptom that the person may even put the painful part in ice water to try to get some relief. Heat makes the pain worse - even the heat built up under bed covers will aggravate them so they are liable to stick the affected part out. The area around the joint might be bruised-looking, or mottled and blotchy, or bluish and pale. They may have the feeling that the pain and stiffness is ascending up the affected limb.
Lycopodium
This remedy has an affinity for the right side in general, so consider Lycopodium for right-sided gouty afflictions, but don’t discount it if the gout attack is on the left side. This is one of the primary remedies for gout in the fingers or the knees, specifically. When moving the affected joint, it is more painful at the beginning of movement, but gets less painful as motion continues. The pains may have a tearing quality to them.
Arnica
The person needing Arnica may have a fear of being touched during a gout attack, so if they flinch or move into a protective gesture when anyone gets near their sore joint, think of this remedy. The pain they experience feels as if they were bruised or beaten up, ‘like someone punched me in my joint’, or like the joint was sprained. They will typically wish to be left alone during the height of their pain. They may be unable to get comfortable because it feels as if the bed or the pillows or the chair they are resting on are too hard or lumpy.
Colchicum
This is one of most often used and helpful remedies in a gout attack. The person needing Colchicum will likely be irritable from the pains. Another key indicator for this remedy is if the sufferer is quite sensitive to smells. They are worse from the slightest of touches and the slightest movement. They feel better from warmth. If the gout attack is associated with flatulence or nausea think of Colchicum. The gouty joint will likely be red, hot, and swollen. They may describe the pain as ‘tearing’ and the pain is usually worse in the evening and at night. As it is one of the most indicated remedies in gout, think of it near the top of your list, especially if the symptoms are too vague to discern another remedy specifically.
Stellaria Media
This remedy has an affinity for gout in the finger joints. Enlarged, painful joints with stiffness are common in this remedy. Sharp shifting pain is a feature, and the pains are worse for motion but also better for continued motion. This remedy is less often indicated than many others, but if the gout is in the finger joints and the person or the affected joints feel both worse AND better for movement, then this remedy might be useful. It is sometimes described as having the motion modalities of both Bryonia (worse from the slightest motion) and Rhus Tox (better from continued motion) combined into the same remedy.
Bryonia
You will recognize this remedy because any slight motion aggravates the condition and causes more pain. The pain is reduced by resting the affected area completely motionless. Pressure may help the pain; this is uncharacteristic in gout so if this symptom is present pay attention to it and consider Bryonia. If Bryonia is needed, thirstiness may accompany the attack and the person may drink large quantities of liquid at spaced out intervals. Heat will aggravate the condition and you will see the characteristic swelling, heat, and stiffness of the joint. Bryonia patients are typically irritable and want to be left alone (except wanting you to refill their water glass at intervals).
Calcarea Carbonica
If this remedy is needed the gout attack may be accompanied by feelings of weakness or tiredness in the affected joint or area. It is often helpful if gout appears in the knees, so consider this remedy if the knees area affected and they feel weak.
Agnus Castus
There are some strong mental and emotional symptoms associated with this remedy while experiencing an ailment - despair and anxiety about their health, a fear that death is near, and a sense that they’ve wasted their life feature in this remedy. If impotency occurs with the gout or if the person also happens to suffer from impotency, this remedy is worth considering. This remedy is often helpful in gout attacks of people who have had issues with gout for a long time, and they may have gouty nodosities built up, especially in the fingers, over time.
Calcarea Phosphorica
In this remedy, the gouty joint will feel most stiff in the morning, and movement will help the stiff feeling. A tired feeling in the limb affected by gout is common, as is the pain being pinpointable to a small area. This remedy is known for a ‘glass half empty’ attitude, so the sufferer may be complaining a lot (about anything) while the gout attack is active. Typically, people needing Calc Phos are sensitive to cold weather, so if a gout attack comes on after cold exposure think of this remedy. Also, if gout attacks routinely occur in winter this might be a good remedy. They will likely have an aversion to open air during the attack, and want to stay inside with the windows shut.
Sabina
If Sabina is needed, the affected area will likely look swollen, red, and shiny. The person may describe stitching pains in the joint and the pain will be worse for movement. The experience of the symptoms will generally be worse in a warm room and better with some air flow. Their mood may be anxious and sensitive. This remedy also has the unusual symptom of not being able to tolerate music, it aggravates the person; so if this symptom appears during a gout attack you would think of Sabina.
Urtica Urens
This remedy is not as commonly needed as some of the others, but if the specific symptoms match, it is very helpful in an acute gout attack. The symptoms are typically worse for cold bathing and cold air in Urtica Urens. If gout is accompanied by hives or urticaria and red blotches on the skin this remedy may be particularly useful. The sensations the sufferer has may be: itching, burning, stinging, and formication (feeling like insects are crawling on or under the skin. And, notably, if gout attacks return at about the same time every year, consider this remedy.
Formica Rufa
This remedy presents with wandering pains, and pains that come on suddenly accompanied by perspiration and restlessness. The feeling may be one of stiffness in the joint, with a feeling of strained or torn muscles around the joint. There may be little red spots on the skin accompanying the gout attack, and these may itch or burn. There are neuralgic pains of the joint during the gout flare up. This remedy may be particularly useful if there are repeated episodes of gout, but always in a different joint.
Benzoic Acid
This remedy is often thought of when gout attacks are associated with or alternate with urinary problems. These issues can include: offensive urine smell, dark coloured urine, sediment in the urine, or a history of kidney stones. With this remedy, there is a tendency for gout attacks to be in the knee or big toe, and more likely on the right side. Another distinctive symptom that indicates Benzoic Acid is if the tongue is cracked, ulcerated, or spongy in association with a gout flare up.
